Outpatient Rehabilitation Portal
Good Shepherd Rehabilitation’s Outpatient Patient Portal provides convenient access to all of your medical information. New and existing Outpatient Rehabilitation patients can create an account on the portal.
- Existing Patients: You should have received an email in early November 2024 with details on how to create your account. If you did not receive the email or have questions, please email [email protected] so that we can assist you.
- New Patients: Once you schedule an Outpatient Rehabilitation evaluation with us, you will receive a Patient Portal Welcome email that contains your username, initial password and link to the portal.
